Dark circles can also be corrected with the refined technique of lipostructure. It is a surgical procedure that requires good technique and skill. It is actually two operations in one: first a small liposuction with a microcannula to harvest the fat, which is then washed to remove unwanted debris, prepared and re-injected into the lower eyelid. Because the area is so delicate, the technique must be performed correctly: many passes are made, each placing a tiny amount of fat, to avoid unsightly lumps.
The fat must be placed beneath the orbicularis muscle, which requires good surgical technique. Patients should know that partial reabsorption of the fat can occur and that its extent cannot be predicted. Statistically, about 25% of patients may need a touch-up later. An alternative for suitable patients is the tear trough filler.
